Central Basildon Primary Care Network (PCN) is delighted to announce that we have been shortlisted for the prestigious PCN of the Year award at the General Practice Awards for the second time, in recognition of our innovative diabetes improvement initiative, Project FFACE.
This achievement recognises the dedication, collaboration and hard work of our healthcare professionals across our eight GP practices, who are committed to improving patient care and making a meaningful difference to the health and wellbeing of our local community.
What is Project FFACE?
Project FFACE is a diabetes improvement initiative developed by Central Basildon PCN to help more patients living with diabetes receive their essential annual health checks, known as the Diabetes 8 Care Processes.
These checks help healthcare professionals monitor diabetes, identify potential complications at an early stage and support patients in managing their condition effectively.
Through Project FFACE, our PCN has introduced a more coordinated and patient-centred approach to diabetes care, bringing essential checks together into convenient, one-stop clinics.
This approach helps reduce the need for multiple appointments, improves access to care and makes it easier for patients to complete their annual diabetes reviews.
